In the [midi] section of dosbox config:
mididevice=alsa midiconfig=128:0 mpu401=intelligent
and then start timidity -iA before dosbox starts. Make sure that timidity -iA says it has bound to 128:0, otherwise adjust the dosbox config accordingly.
